Knowing what to expect with your healthcare recovery is important. We have created a fill-in Postpartum Healthcare Plan tool for your care team members to write down your postpartum medical, health care, and recovery plan.
We hope care team members will use this during your healthcare visit(s) to discuss your medical care, information about any treatment and conditions, information about provider referrals, talk about what is important to you, help you find any resources or support you need, and know actions to take once you’re home.
For Care Team Members: Consider introducing this tool during their final prenatal visits or at discharge so mom and their village are prepared for healthcare at home and aware of their team of providers, local resources, and more.
