Your Postpartum Healthcare Plan

< Back to Resources Home

During healthcare visits and at discharge, providers cover a lot of information. It can be difficult to remember and adhere to some of their council once you are home in the throes of it!

Knowing what to expect with your healthcare recovery is important. We have created a fill-in Postpartum Healthcare Plan tool for your care team members to write down your postpartum medical, health care, and recovery plan.

We hope care team members will use this during your healthcare visit(s) to discuss your medical care, information about any treatment and conditions, information about provider referrals, talk about what is important to you, help you find any resources or support you need, and know actions to take once you’re home.

Your Postpartum Health & Recovery Plan

For Care Team Members: Consider introducing this tool during their final prenatal visits or at discharge so mom and their village are prepared for healthcare at home and aware of their team of providers, local resources, and more.

Know that some health problems after giving birth are life-threatening and can happen up to a year after you have a baby? Recognizing when something isn’t right and taking urgent action could save Mom’s life. Here is a brief training for family members/communities to know what to look for and what to do if a birthing person is experiencing warning signs. Find more maternal health warning sign resources, including reels, additional languages, and materials here.

Welcome to NewMomHealth.com™

We envision a world where every woman receives the social, emotional, physical, and economic support she needs to successfully transition through the postpartum period and into her new identity and life as a mother. Health care systems, businesses, and society should value and respect women not only for what they bring to families, communities, and nations but also for who they are in and of themselves.
