Improving Postpartum Care for NC Parents

IMPLEMENTATION SUPPORT AND TECHNICAL ASSISTANCE OPPORTUNITY

Join us! The 4th Trimester Project team at UNC-Chapel Hill has support from the HRSA-funded NC Maternal Health Innovation Initiative to collaborate with North Carolina clinics to improve care for postpartum people and their families. We are able to work with a total of 10 sites (clinics or health care systems) across each of the perinatal regions – to continue testing and implementing components of the 4th Trimester Project Postpartum Toolkit. Postpartum Toolkit Documents: You can access Postpartum Toolkit materials here. These are open-access tools for healthcare teams to use with pregnant and postpartum people.

Community of Practice: The Community of Practice is open to all healthcare professionals in North Carolina. The content discussed will be guided by participant interests and needs. The goal is to learn with and from each other. Register here. To suggest a topic or speaker, email SuzanneW@med.unc.edu.

Office Hours: For our NC MHI sites, the 4th Trimester team offers virtual office hours 2nd Mondays of each month at noon. We are here to walk with you around considering toolkit items, integrating them into practice, and evaluating the effectiveness for continual system strengthening.
