Postpartum Visit Checklist

The postpartum visit is an important time to listen and thoughtfully engage with new parents. To support patient-centered care, the 4th Trimester Project team has tested, adapted, and launched the postpartum visit checklist. This tool is intended for a birthing person to fill out and share with the clinic staff so their provider can prioritize the information and topics mom wants to learn about and cover during the postpartum visit.

This is an important touchpoint, and opportunity to care for mom. It is important to make the time count. This postpartum visit checklist gives new parents a chance to think about their top questions as well as to learn about important information and resources for support.

Care teams can use this guide for tips and suggestions to introduce and use the tool in practice. This postpartum visit checklist is intended for clinics to know what the birthing person would like to talk about and prioritize the visit towards what is important for them to engage and learn about.
